There is good news, and there is bad news. The good news is that in my experience, younger physicians generally are much more concerned about the cost of clinical tests and treatments, and about justly distributing finite medical resources, than were those who practiced medicine in the fee-for-service era. The bad news has at least three components. First, with respect to medically nonbeneficial treatment in the ICU, managed care has not yet given evidence of wanting to put the brakes on (...) unrealistic family demands for aggressive medical interventions. Second, managed care is frustrating many healthcare professionals as well as patients. And third, managed care has no apparent interest in addressing, and may even have contributed to, the problem of medical indigence. Let me develop these propositions more fully. (shrink)

The basis of science is the hypothetico-deductive method and the recording of experiments in sufficient detail to enable reproducibility. We report the development of Robot Scientist "Adam," which advances the automation of both. Adam has autonomously generated functional genomics hypotheses about the yeast Saccharomyces cerevisiae and experimentally tested these hypotheses by using laboratory automation. We have confirmed Adam's conclusions through manual experiments. To describe Adam's research, we have developed an ontology and logical language. The resulting formalization involves over 10,000 different (...) research units in a nested treelike structure, 10 levels deep, that relates the 6.6 million biomass measurements to their logical description. This formalization describes how a machine contributed to scientific knowledge. (shrink)

From childhood through to adulthood, retirement and finally death, _The Economic Psychology of Everyday Life_ uniquely explores the economic problems all individuals have to solve across the course of their lives. Webley, Burgoyne, Lea and Young begin by introducing the concept of economic behaviour and its study. They then examine the main economic issues faced at each life stage, including: * the impact of advertising on children * buying a first house and setting up home * changing family roles (...) and gender-linked inequality * redundancy and unemployment * coping on a pension * obituaries, wills and inheritance. Finally they draw together the commonalties of economic problems across the lifespan, discuss generational and cultural changes in economic behaviour, and examine the significance of other, non-economic constraints, upon individuals. _The Economic Psychology of Everyday Life_ provides a much-needed comprehensive and accessible guide to economic psychology which will be of great interest to researchers and students. Medical technologies and assistive devices such as ventilators and power wheelchairs are designed to sustain life and/or improve functionality but they can also contribute to stigmatization and social exclusion. In this paper, drawing from a study of ten men with Duchenne muscular dystrophy, we explore the complex social processes that mediate the lives of persons who are dependent on multiple medical and assistive technologies. In doing so we consider the embodied and emplaced nature of disability and how life is lived (...) through bodies coupled with technologies and experienced as 'techno-body-subjects in situ'. The stories people tell in the course of conversations are both implicated in and distinct from the occasions on which they are told. Their implication is a matter of context; their distinctness is a matter of frame. Contexts show up as continuities between stories, conversations, and storytelling occasions; frames mark discontinuities. By virtue of their frames, stories can be identified as a different order of event from the conversations in which they are enclaves and from the occasions of their telling, (...) an event with its own conventions of presentation. Stories are neither natural utterances in conversations nor are they ordinary events in everyday life. They are recountings of such events in the course of such conversations. Stories thus constitute a separate universe of discourse, a storyrealm embedded in the realm of conversation. The storyrealm, that segment of narrative discourse within conversation, in turn directs attention to a third realm, the realm of the events the story is about or taleworld. Events in the taleworld are framed by the storyrealm, itself framed by the realm of conversation. (shrink)

Family traditions take a somatic turn in a therapeutic practice that focuses on how bodies are passed down in families, not as assemblages of biological traits enjoined on the bodies of children by parents but as intentional fabrications devised by children out of the bodies of parents. Somatic psychology holds that parents offer children models of how to be embodied in the form of bodily attitudes. The body shapes I imitate and resist at every stage of life arise out of (...) the memory of the past, incarnate memory in the present, and project the memorial body toward its future. Taking up the techniques by which a person makes her body, the somatic psychologist induces her to unmake and remake it. This is a study of how the flesh remembers. (shrink)

Characterizations of an (∈, ∈)-neutrosophic ideal are considered. Any ideal in a BCK/BCI-algebra will be realized as level neutrosophic ideals of some (∈, ∈)-neutrosophic ideal. The relation between (∈, ∈)-neutrosophic ideal and (∈, ∈)-neutrosophic subalgebra in a BCK-algebra is discussed. Conditions for an (∈, ∈)-neutrosophic subalgebra to be a (∈, ∈)-neutrosophic ideal are provided. Using a collection of ideals in a BCK/BCI-algebra, an (∈, ∈)-neutrosophic ideal is established. Después de analizar las actuales formas de producción en Brasil, sostienen que la expansión de éstos presionará indirectamente hacia la deforestación, contribuyendo así aún más al calentamiento global. Señalan lo falaz del discurso que justifica la expansión de los biocombustibles por su aporte a la generación de empleo. Sostienen que es fundamental incorporar los requerimentos de sostenibilidad en la cadena de producción para (...) garantizar competitividad a los países potencialmente exportadores, pero que aún así esa solución es limitada ya que todas las energías alternativas y renovables juntas, no suplirían las proyecciones del patrón de consumo mundial. Past research has found that skin colouration, particularly facial redness, influences the perceived health and emotional state of target individuals. In the current work, we explore several extensions of this past research. In Experiment 1, we manipulated facial redness incrementally on neutral and angry faces and had participants rate each face for anger and health. Different red effects emerged, as perceived anger increased in a linear manner as facial redness increased. Health ratings instead showed a curvilinear trend, as both extreme (...) paleness and redness were rated as less healthy than moderate levels of red. Experiment 2 replicated and extended these findings by manipulating the masculinity of both angry and neutral faces that varied in redness. The results found the effect of red on perceived anger and health was moderated by masculine face structure. Collectively, these results show that facial redness has context dependent effects that vary based on facial expression, appearance, and differentially impact ratings of emotional states and health. Emotion expressions convey valuable information about others’ internal states and likely behaviours. Accurately identifying expressions is critical for social interactions, but so is perceiver confidence when decoding expressions. Even if a perceiver correctly labels an expression, uncertainty may impair appropriate behavioural responses and create uncomfortable interactions. Past research has found that perceivers report greater confidence when identifying emotions displayed by cultural ingroup members, an effect attributed to greater perceptual skill and familiarity with own-culture than other-culture faces. However, the current research (...) presents novel evidence for an ingroup advantage in emotion decoding confidence across arbitrary group boundaries that hold culture constant. In two experiments using different stimulus sets participants not only labeled minimal ingroup expressions more accurately, but did so with greater confidence. These results offer novel evidence that ingroup advantages in emotion decoding confidence stem partly from social-cognitive processes. (shrink)

The aim of this paper is to challenge the claim that the neural activity commonly referred to as 'readiness potential' constitutes evidence for the unconscious initiation of action. Although I accept that such neural activity seriously challenges the commonly held view that one's sense of volition is causally efficacious, I nevertheless contend that much of our everyday engagement with the world is consciously initiated. Thus, a distinction is made between awareness and what the awareness is of: the latter constituting the (...) conscious decision to act in accordance with one's goal, or what I have termed intentional project. Initiation of an action in accordance with one's intentional project grounds the action in meaning, something that would be lacking in an exclusively unconscious decision to act. (shrink)

All-encompassing text examines every aspect of coma from neurochemistry, monitoring, and treatments to prognostic factors.

We have synthesized a 582,970-base pair Mycoplasma genitalium genome. This synthetic genome, named M. genitalium JCVI-1.0, contains all the genes of wild-type M. genitalium G37 except MG408, which was disrupted by an antibiotic marker to block pathogenicity and to allow for selection. To identify the genome as synthetic, we inserted "watermarks" at intergenic sites known to tolerate transposon insertions. Overlapping "cassettes" of 5 to 7 kilobases (kb), assembled from chemically synthesized oligonucleotides, were joined by in vitro recombination to produce intermediate (...) assemblies of approximately 24 kb, 72 kb ("1/8 genome"), and 144 kb ("1/4 genome"), which were all cloned as bacterial artificial chromosomes in Escherichia coli. Most of these intermediate clones were sequenced, and clones of all four 1/4 genomes with the correct sequence were identified. The complete synthetic genome was assembled by transformation-associated recombination cloning in the yeast Saccharomyces cerevisiae, then isolated and sequenced. A clone with the correct sequence was identified. The methods described here will be generally useful for constructing large DNA molecules from chemically synthesized pieces and also from combinations of natural and synthetic DNA segments. 10.1126/science.1151721. Glioblastoma is a brain tumor condition marked by rapid neurological and clinical demise, resulting in disproportionate disability for those affected. Caring for this group of patients is complex, intense, multidisciplinary in nature, and fraught with the need for expensive treatments, surveillance imaging, physician follow-up, and rehabilitative, psychological, and social support interventions. Few of these patients return to the workforce for any meaningful time frame, and because of the enormity of the financial burden that patients, their caregivers, and society face, utilization (...) reviews become the focus of ethical scrutiny. While primary malignant brain tumors account for only two percent of all adult cancers, these neoplasms cause a disproportionate amount of cancer-related disabilities and death. The five-year survival rates for brain tumors are the third lowest among all types of cancer. Malignant gliomas comprise the most common types of primary central nervous system tumors and have a combined incidence of five to eight cases per 100,000 people. The median survival rate of conservatively treated patients with malignant gliomas is 14 weeks; (...) with surgical resection alone, 20 weeks; with surgery and radiation, 36 weeks; and with the addition of newer biochemotherapies such as temozolomide and bevacizumab, upward of 14-18 months.The profound cost of caring for terminally ill patients with primary malignant brain tumors raises ethical considerations for the American public; the stewardship of health care dollars for the population at large maintains a juxtaposed tension against a dynamic, necessary balance of hope, care, rehabilitation and research efforts for affected patients and their advocates.
